Permit to Work Register: What It Is and How to Maintain It
A permit to work register is one of the most consistently under-maintained parts of a permit-to-work system. Sites spend significant effort on permit forms and pre-work checks, then allow the register — the log of all permits issued and closed — to become a pile of incomplete papers in a site cabin or an unindexed folder on a server.
This guide explains what a permit-to-work register is, what makes it legally and operationally effective, and the common failure modes that undermine systems that work well on the front end but fall down when inspectors ask for records.
What is a permit to work register?
A permit-to-work register is a running record of every permit issued, active, and closed on a site. At any point, it should tell you: how many permits are currently live, what work is being done under each, and what has been completed and signed off.
The register serves several distinct functions:
Operational visibility. If you are a site manager with three permits live — a crane lift in Zone A, hot work in Zone B, and a confined space entry in Tank 4 — the register tells you that at a glance. This matters when a third permit is issued that might interact with an active one (two trades working in the same zone), or when an emergency requires you to know immediately who is in a permitted area and what they are doing.
Audit trail. When the HSE inspects, or when an incident investigation starts, the permit register is the first document requested. It establishes what was authorised, who authorised it, when work started, and when permits were formally closed. A register with gaps or unsigned close-outs is a significant enforcement liability.
Legal record. CDM 2015 regulation 12 requires the principal designer to maintain a health and safety file containing "information relating to the project which is likely to be needed during any subsequent project to ensure the health and safety of any person." Completed permits are part of that file — evidence that high-risk work was controlled. Under CDM 2015 regulation 12(10), at the end of the project the health and safety file must be passed to the client.
HSE guidance in HSG250 (Permit-to-Work Systems) treats the permit register as a core component of any permit system — not an optional add-on.
What the register must contain
Each entry in the register should capture, at minimum:
Permit details:
- Permit number (sequential — this is the cross-reference when a permit is mentioned in an incident report or investigation)
- Date and time of issue
- Permit type (hot work, confined space, electrical isolation, working at height, etc.)
- Location — specific enough that someone could find the work area from the register entry alone
- Description of work (not "maintenance" — the specific task: "replacement of pump P4 impeller, east pump room")
- Permit holder name and employer
Status tracking:
- Date and time the permit was returned and closed
- Name and signature of the person who closed the permit
- For hot work permits: confirmation that fire watch was completed and at what time
- For confined space permits: confirmation that all persons are out of the space
Supporting documents referenced:
- Risk assessment reference
- Method statement reference
- For crane lifts: lift plan reference
- For electrical isolation: circuit test results or isolation record reference
Common register failures — and what they signal
Permits with no close-out entry. This is the single most common failure. Work finishes, the team moves on, and the permit stays "open" in the register indefinitely. In an audit, every unclosed permit is a question: was close-out actually completed? If not, were the controls (fire watch, re-instatement of isolations, guards replaced) actually completed?
A high close-out completion rate — ideally near 100% — is a sign of a healthy permit system. Persistent unclosed permits indicate a systemic problem: either the close-out step is not being followed, or the register is not being kept in real time.
Missing permit numbers. If permits are numbered but the register has gaps (permits 47, 48, 50, 51 — no 49), this raises the question: what happened on permit 49? Was it issued and the entry lost? Was it never issued? In an investigation, gaps create the impression that the register has been edited rather than maintained.
"Open" permits from previous days. If permit 47 was issued yesterday and is still showing as open today, either the work is legitimately ongoing (which should be documented), the permit was not closed when it should have been, or the work ran beyond its permitted time. Any of these is a finding in an audit.
No location information. A register entry that says "hot work" with no location is operationally useless and legally inadequate. If there is a fire tonight, the register cannot tell an incident investigator where the hot work was.
Permit issued but no risk assessment referenced. If the register has no risk assessment reference, the trail from "authorised permit" back to "identified hazards and controls" is broken. In a prosecution, this gap is hard to explain.
Paper vs digital permit registers
A paper register — a site logbook or a dedicated PTW ledger — works if it is maintained consistently. The structural problems with paper are: entries depend on individual discipline, close-outs are the step most likely to be missed, and searching a paper register for all permits in a specific area over a 12-month period is time-consuming.
A digital permit system maintains the register automatically — each permit issued creates a register entry, and the close-out step on the digital form updates the register in real time. The operational visibility benefit (who is live at this moment?) becomes immediate rather than requiring someone to walk to the site cabin and check the logbook.
The register is one of the key arguments for digital permit management on complex sites: the discipline of maintaining it is built into the workflow rather than depending on someone remembering to write it up.
How long must permit records be kept?
There is no single statutory retention period for permit records across all industries. The practical answer depends on:
- The nature of the work and the site
- Whether the site is subject to CDM 2015 (permits should be retained for the life of the project, and completed permits may form part of the health and safety file)
- Whether any incidents occurred under permitted work (retain indefinitely if litigation or investigation is possible)
- Industry sector guidance (oil and gas, chemicals, and nuclear have sector-specific retention requirements)
As a general practice, retaining completed permits for a minimum of five years satisfies most industry-standard expectations for construction and general industry. For sites with significant life-safety risk profiles (major hazard sites under COMAH), sector-specific requirements may require longer retention.
Integrating the register with your permit audit
A register is only as useful as the discipline applied to maintaining it. The most effective way to maintain that discipline is to make the register a standing item in your permit-to-work audit — specifically:
- Weekly spot-check: are all currently-open permits genuinely live?
- Monthly review: are close-out entries complete for all permits issued in the previous month? Is the close-out completion rate satisfactory?
- Quarterly audit: does the register align with what was actually happening on site? Are there categories of work where permits should have been issued but weren't?
